A Clinician’s Guide to Microneedling: What It Is, How It Works, and Who It’s For

As a Dermal Clinician, I am focused on treatments that are backed by science and deliver real, long-term change. Of all the advanced tools we have, one of the most effective and proven is microneedling, also known as Collagen Induction Therapy (CIT).

I understand that the word “needling” can sound intimidating, but this is one of the most transformative treatments we can perform. It’s a safe, controlled, and minimally invasive procedure that uses your skin’s own natural healing power to rejuvenate itself from the inside out.

What is Microneedling?

Clinical microneedling is a procedure where a medical-grade device, equipped with fine, sterile, single-use needles, is passed over the skin. This creates thousands of precise, vertical micro-channels in the skin.

These “micro-injuries” are the key. They are so small that they don’t cause a real wound, but they are just enough to signal your body to “wake up” and start a cascade of healing. This healing response is what leads to total skin rejuvenation.

How Does Microneedling Actually Work? The Science.

When the device creates these micro-channels, it triggers a three-phase wound-healing response:

  1. Phase 1: Inflammation. Immediately after the treatment, your skin will be red and flushed. This is a good thing! Your immune system is sending blood flow and growth factors to the area to begin the repair process.
  2. Phase 2: Proliferation. Over the next few days, your skin gets to work. Cells called fibroblasts, which are your body’s “collagen factories,” are signaled to start producing brand new, high-quality collagen and elastin.
  3. Phase 3: Remodelling. This is where the magic happens. Over the next 4-12 weeks (and even months), your skin lays down this new, strong collagen matrix. This is what smooths scars, plumps fine lines, and strengthens the skin.

We are essentially tricking the skin into rebuilding itself, but in a stronger, smoother, and more organised way than before.

What Can Microneedling Treat? (Who Is It For?)

Because microneedling works at this deep, structural level, it is a versatile treatment for a huge range of concerns. I recommend it for clients wanting to improve:

  • Acne Scarring: Especially pitted or atrophic (indented) scars. It helps to break down old scar tissue and rebuild the area with new, healthy collagen.
  • Fine Lines & Wrinkles: The new collagen and elastin plump the skin, softening the appearance of ageing.
  • Uneven Skin Texture: It smooths a rough or “bumpy” texture and can visibly reduce the appearance of enlarged pores.
  • Pigmentation & Sun Damage: It helps to break up pigment clusters and normalise the pigment-producing cells (melanocytes), leading to a more even skin tone.

What to Expect During Your Microneedling Treatment

First, we will always start with a thorough consultation to ensure you are a good candidate. This treatment is not suitable for those with active acne, infections, or certain skin conditions. Usually you will need to go through a period of preparation before we can begin microneedling, however, this will depend on your current skincare products and the condition that your skin is in.

  1. The Cleanse: Your skin will be deeply cleansed and sterilised.
  2. The Procedure: I will work methodically across the skin with the device. Most clients describe the sensation as a “prickly” or “vibrating” feeling. It’s generally very tolerable and fast.
  3. Post-Treatment: Immediately after, your skin will be red and feel warm, similar to a moderate sunburn. This is the “inflammation” phase starting, and it’s exactly what we want to see. This redness typically subsides significantly within 24-48 hours.

I will provide you with a strict aftercare plan, which includes what to use, what not to use (like active ingredients or makeup), and the non-negotiable use of sunscreen.

You will see an initial “glow” and plumpness within the first week, but the true, lasting results appear from week 4 onwards as that new collagen builds. For best results, I almost always recommend a course of 3-6 treatments, spaced 4-6 weeks apart.
